We work closely with your physician to develop a specific care plan tailored to meet all of your nursing needs. Each of our nurses is highly skilled in disease management, observation, assessment, and education, catheter care, wound and ostomy care, IV infusion and injections, tube feedings and care, nutritional assessment and specialized care for other chronic conditions.PHYSICAL THERAPY:Our highly qualified physical therapists provide services that will help restore function, improve mobility, relieve pain and prevent or limit permanent physical disabilities that you may have incurred from injuries or disease. We work with your physician to develop and implement a customized plan specific to your condition. We provide gait and balance exercise and training, strength and endurance training, individualized home exercise program, adaptive equipment assessment, safety training, and have a specialized fallprevention program. OCCUPATIONAL THERAPY:Our occupational therapists aim to improve your ability for daily functions and work environment tasks by providing you with the skills needed for the performance of daily living. Our therapists will help you to develop, recover or maintain motor functions and reasoning abilities. Our services include therapies for activities of daily living, strength and endurance training, perceptual and fine motor training, adaptive equipment assessment, splinting and home safety program.SPEECH THERAPY:Pacifica's speech therapists concentrate on patients with impaired speech, language and voice disorders. Our therapists assess, treat, and help to prevent speech, language, voice, and swallowing fluency. We provide treatment for voice disorders and speech articulation, such as inappropriate pitch, or harsh voice, dysphasia, inability to produce speech sounds or produce them clearly, language disorders, cognitive disorder treatment, dysphasia and swallowing treatment.MEDICAL SOCIAL WORKERS:Our medical social workers offer compassionate counseling and advocacy as they tackle your social, financial, and medical needs. We provide you with community resources and coping techniques that will help you with personal, emotional and financial problems, and assistance with long or short-term placement in a care facility, if appropriate.HOME HEALTH AIDE:Our certified home health aides lend a helping hand in daily activities such as personal care, hygiene needs including grooming and bathing, transfers, walking, light housekeeping, meal preparation, and compliance with doctor's orders and medication management.
